How Radial Pulse Wave Shockwave Therapy Works

Olomana Mobile Physical Therapy delivers Extracorporeal Shockwave Therapy using Radial Pulse Wave (RPW) technology, applied through the Chattanooga Mobile RPW2 — a portable, hospital-grade device that makes this treatment possible right in your home. It can be incorporated directly into a PT or personal training session, or your initial evaluation, upon request. Olomana Mobile PT is currently the only practice in Hawaiʻi offering true mobile shockwave therapy.

What It Actually Does

Despite the name, no electricity is involved. The device sends focused pulses of acoustic (sound) pressure through the skin into the injured tissue below. That mechanical energy kick-starts the body's own healing response in a few ways:

  • Increases blood flow to the area by encouraging the growth of new small blood vessels — something chronically injured tendons often lack on their own.
  • Stimulates the cells that rebuild tissue (fibroblasts), helping lay down new, stronger collagen fibers in place of damaged ones.
  • Breaks down calcific buildup and old scar tissue or adhesions that have hardened in long-standing injuries.
  • Reduces pain by temporarily overwhelming local nerve endings (a process called hyperstimulation analgesia) and, over a series of sessions, desensitizing overactive pain receptors.

In short, it restarts a healing process that has stalled — which is why it works best on chronic injuries that have lingered for weeks or months, rather than a fresh, acute injury.

Conditions It Can Treat

  • Achilles tendinosis
  • Plantar fasciitis
  • Lateral epicondylitis (tennis elbow)
  • Medial epicondylitis (golfer's elbow)
  • Rotator cuff tendinosis
  • Patellar tendinosis (jumper's knee)
  • Hip & greater trochanteric bursitis
  • Stubborn muscle knots & myofascial trigger points
  • Scar tissue & soft-tissue adhesions
Olomana Mobile Physical Therapy is currently the only company in Hawaiʻi offering mobile shockwave therapy — bringing this specialized, hospital-grade treatment directly to your door.

Care in Motion

What a session actually looks like

Your physical therapy comes to you. I arrive at your home with the equipment needed for your session—including a treatment table, exercise equipment, and mobile RPW shockwave therapy when appropriate.

Your first visit includes a comprehensive evaluation to understand your condition, identify the factors contributing to your symptoms, and establish a personalized treatment plan. Evaluation and treatment begin during the same visit.

Follow-up visits typically include reassessment of your progress followed by hands-on treatment, targeted exercises, mobility work, strength and movement training, and RPW shockwave therapy when indicated.

Each session is tailored to what you need that day and typically lasts 60–90 minutes. Because this is concierge, direct-access physical therapy, treatment isn't restricted to a specific diagnosis or limited number of visits. If a new injury or concern comes up, it can be evaluated and addressed as part of your ongoing care.

You'll leave each session with a clear plan—including exercises and recommendations to help you continue making progress between visits.
